To be considered for membership in CMEN-LA you must
1) be at least 21 years old, 2) read and agree to follow the event guidelines listed on the website,
3) attend at least two CMEN-LA events as a guest, and 4) complete this Questionnaire. Annual dues
are curently just $7.00 per person. Do not send money with this Questionnaire. Dues are collected
